If you are applying to foster an animal, please complete and sign the Foster Care Information & Expectations Form in addition to the Contact Information Form located on this page. If you are applying to adopt an animal, please complete and sign the Responsible Pet Ownership & Non-Breeding Agreement in addition to the Contact Information Form located on this page. Please note that applicants are only required to complete the forms relevant to their application type.

If you are surrendering a dog, cat, or other pet, we understand this may be a difficult decision. We kindly ask that you be as honest and detailed as possible about the reason for surrender so we can provide the best possible care and placement for your animal.

Whenever possible, we ask that you provide adequate notice to allow us time to arrange foster care or transportation.

If you are able, please send your pet with any available supplies such as food, toys, bedding, medication, or veterinary records. If supplies cannot be provided, a surrender fee may be requested to help cover the cost of essential care items. If this may be difficult for you, please let us know when submitting your application so we can discuss options with you.
